SELF-SCREENING QUESTIONNAIRE FOR THOSE SEEKING FUNDING

Check all that apply to your organization and program

 

 

SECTION ONE: Age and Characteristics of Persons Served

Does your program

    • Promote the health and well-being of underserved youth?

    • Seek to support the needs of young persons between the ages of 9 and 25?

We need at least one YES for you to proceed to Section Two.

 

 

SECTION TWO: Characteristics of your Program’s Need

Is the need for which you are seeking funds:

     • a new program for your organization?

     • an effort to stabilize a program started in the last three years?

     • an expansion of a tested program, which seeks to spread to new areas?

     • an effort to rescue a faltering program, which needs funds temporarily to get back on its feet?

We need at least one YES for you to proceed to Section Three

 

 

SECTION THREE: Program Content Questions:

Does your program

      • Involve young persons in the planning and/or execution of the program?

      • Encourage initiatives, or innovative and experimental existing programs,

         which empower youth, cultivate skills, and encourage healthy living?

      • Equip youth with Leadership Training?

      • Promote ministries that in their aim and execution are compatible with the Christian view of life

         and society, which values all creation as the work of God and seeks reconciliation

         and love among human beings.

      • Provide service-oriented or community building efforts (e.g. paid or volunteer group projects

        for youth, especially during the summer? youth projects providing assistance to the elderly,

        handicapped, etc.? youth environmental projects, such as education or cleanup, vocational

        assistance such as job counseling, employment of youth, group volunteer opportunities and so on)?

We need at least one YES for you to proceed to Section Four.

 

 

SECTION FOUR: Financial and Geographic Questions

    • Is your program for youth living in Massachusetts east of Worcester County?

    • Do you or your fiscal agent have an IRS 501(c)(3) tax exempt status?

    • Will our grant monies go to supporting human talent rather than buildings or overhead costs?

    • Is your program budget less than $350,000?

    • Are you able to provide financial information for both current and prospective funding sources

       not only for your program but also for your sponsoring institution?

You must answer YES to all five questions above in order for the Board of Church Home Society to consider your grant request.

 

 

If you have answered with at least one YES each in Sections One, Two, and Three, and YES to all five questions in Section Four, congratulations. You may now proceed to our Grant Application Form by clicking here or by calling Mrs. Roberta Tripp, the administrator,  at 617-482-5328 and requesting that  the form be sent to you by mail.
